The Effect of Non-steroidal Anti-inflammatory Drugs in Upper Gastrointestinal Bleeding

Abstract

|
Objective: This prospective multi-center, case-control study was conducted to analyze the relationship between nonsteroidal anti-inflammatory drugs (NSAIDs) and upper gastrointestinal bleeding (UGIB) in a Korean population.
Method: A total of 168 pairs of cases (endoscopic evidence of UGIB) and controls (no UGIB on endoscopy), matched for age and gender, were enrolled in six Korean hospitals during a one-year period. Data were collected on NSAIDs use, medical history, medication history, and smoking. The age- and gender-adjusted risk for UGIB with NSAIDs use was compared between the case and control groups.
Results: The matched odds ratio of UGIB with NSAIDs intake (adjusted for medical history, medication history, and matched for age and gender) was 5.0, for past medical history 5.0, and for history of medication 1.7. There was no significant difference in the site of UGIB between the two groups. Helicobacter pylori infection rates were 61.4% in the case group and 58.2% in the control group. There was no relationship between UGIB and concomitant medication in either group.
Conclusion: This prospective multicenter study indicates that NSAIDs use in a Korean population is associated with a five-fold increased risk of UGIB.
